Responsibilities:
Provide direct patient care in facilities or homes under the license of a RN.
Provide ongoing patient care, and oversight of CNAs.
Establish relationships within the community and case workers at hospitals and rehab facilities Ability to function independently Support the functions of the Agency as required.
Maintain confidentiality of all patient records in accordance with agency policies and procedures.
Perform other duties as assigned by RN or the director of operations.
Required Skills:
Valid LPN license in the state of MA.
Must be CPR certified.
Prior experience in a long-term care or skilled-care environment preferred.
Must be able to perform basic tasks under pressure/with attention to detail.
Ability to change dressings, provide medication management, assess situation, background and initiate interventions as appropriate.
